The government wants to prevent and better support whānau who are survivors of stroke. We’d like to understand how you can be supported:

When you were first diagnosed with your condition, was the information explained to you clearly?

Do you feel you were given the necessary information to share with your whānau?

Were you told all the risks of your condition?

Were you give good information about the treatment plan to manage your condition – like what to eat, exercising, the medication you should take, the options you have?

If you are on medications – how do you find this is working for you? Is it easy for you to access and remember to take? Did the pharmacist explain the medications to you?

When you have needed to go back to the doctor for monitoring your condition, has that been easy for you? How? If not why not?

What else can be done to help you self-manage your condition easier and ensure your condition doesn’t worsen and cause you complications?

The government has a target to improve immunisation rates for all children to 95%. Currently the rates for Māori are very low. We don’t mind if you made a conscious choice not to immunise or not. We are interested in your perspectives on immunisation and services. Possible questions for whānau.

Do you feel you get the right information about immunisation to decide whether to immunise your pēpi / tamaiti or not?

Once you made a decision to proceed with immunisation – did you manage to find someone to do it for your pēpi / tamariki? What barriers or problems occurred for you?

What supports did you get?

Was it easy for you to get to immunisation service? Was it available for you conveniently?

Was the immuniser informative for you and your pēpi? Made you feel comfortable? Let you know what it was for? Give you the information you needed?

What is a good way to remind you about immunisations that are due? IS that how you get notified at the moment?

If you haven’t been – what is preventing you from going? How can things be made easier for you?

Overall - how would you rate the immunisation service you received? And will go back for future immunisations?
